The book and CD are available direct from the John Fletcher Trust, 14
Hamilton Terrace, London NW8 9UG.  Prices for the book are 12 pounds in
the U.K., 13 pounds in Europe, 15 pounds overseas.  Prices for the CD are
10 pounds in the U,K, or 12.50 pounds elsewhere.  All prices include
shipping.  Payment is accepted by International Money Order, Eurocheque,
or a cheque payable at a London bank.
