In my experience - the musical symbol for "take care / watch out"  is a
hand-written pencil circle entered hurriedly by the musician on the part
immediately after an embarrassing cock-up in rehearsal.

An alternative - "keep your eyes open / look ahead because you know you'll
forget" is conventionally a pencil sketch of a pair of eyes or spectacles in
the margin of the sheet.
