Rolf,

Malcolm Forsythe is a trombonist who lives in Edmonton, Alberta, Canada.  He
played principal trombone there for some time and as far as I know he now
composes full time.

The Golyardes Grounde can also be found on one of the first Canadian Brass
Recordings.  It used to be found through the Canadian Music Centre in Toronto,
Montreal or Vancouver.  I believe it is now published.  I will try and find
out who the publisher is.  I did see a copy of it at a local large music shop
here in Vancouver not long ago.
